If a donor arrives at a collection facility for a DOT drug test and they make an unsuccessful attempt to provide a sample (and therefore enters into a shy bladder situation) what should you do as the collector, if after the two hours the donor provides a specimen that is out of temperature range?

The collector should package the initial specimen for shipping and then instruct the donor that they now have to provide a specimen under direct observation within the next hour.

After the laboratory reports a test result, someone (e.g., the employer, a service agent) discovers that the CCF listed the wrong reason for the test (e.g., the CCF says the test was a pre-employment test when it was actually a random test). How is this corrected and by whom?
